As an awardee of the New York State contract for providing Practice Transformation Technical Assistance, HANYS will partner with primary care practices across the state to guide them in achieving New York State Patient-Centered Medical Home (NYS PCMH) recognition. HANYS’ unique experience with practices has positioned us for success in assisting our partners towards transformation. Our experience with practice transformation goes well beyond just understanding the guidelines and how to prepare supporting documents. We also understand the challenges and obstacles, as well as the rewards, associated with true site transformation. It is only true transformation that leads to the sustained outcomes and achievement of the Triple Aim.

Our technical expertise spans a wide variety of electronic medical records (EMR) platforms, and our approach to achieving site transformation is highly customized and focused on the specific needs of the practice. Our team is well versed in process, people, and technology. We apply proven approaches to accomplish transformation, improve outcomes, and enhance patient, staff, and provider satisfaction while bending the cost curve. We accomplish this by engaging staff and system leaders and by following a detailed project plan for the transformation team.
